Tailored Care Management (TCM)

Hope Services, LLC is a certified Care Management Agency. As such we are credentialed to provide Tailored Care Management (TCM) services.  The overall goal of TCM is improved health outcomes for members.  Upon enrollment in a Tailored Plan, members will be auto-enrolled in Tailored Care Management unless:

• They are receiving ACT
• They are residing in ICFIDD homes
• They are a child participating in Care Management for At Risk Children (CMARC) or High Fidelity Wraparound
• They opt out

TCM does not replace any current services, rather it is an additional service that is available to members which can help them navigate their physical and behavioral health needs.
